Annual Fire Door Inspections: What Actually Gets Verified

Fire doors fail inspection more often than almost any other life safety item in a commercial building, and it is rarely because anyone neglected them. It is because a fire door is a listed, tested assembly, and normal building activity slowly turns it into something that was never tested.

Short answer: NFPA 80 requires fire door assemblies to be inspected and tested not less than annually, with a written record retained. On a swinging door the inspection is a defined set of visual checks plus an operational test that the door closes and latches on its own. On a rolling steel door it is a drop test. Most deficiencies are inexpensive to correct once they are identified.

Why a fire door is different from a door

A fire door assembly is listed as a system: the door, the frame, the hinges, the closer, the latching hardware, the glazing, and the anchorage were tested together and labeled together. Change one component and the assembly is no longer the thing that passed the test.

That is why field modification is a failure condition on its own, independent of whether the modification looks harmful. Drilling the door to mount hardware, cutting in a louver, or replacing a closer with an unlabeled one all take the assembly outside its listing.

What is verified on a swinging fire door

The visual portion of the inspection covers a specific list. An inspector confirms that:

Door and frame labels are present, clearly visible, and legible. Painted-over or illegible labels are a deficiency, and they are the single most common one we see.

There are no open holes or breaks in the surfaces of the door or the frame.

No parts on the door or frame are missing or broken.

Where glazing is present, the glazing, vision light frames, and glazing beads are intact and securely fastened.

The door, frame, threshold, and hardware are secure and aligned, with no visible damage.

The hardware installed is labeled for use on that door.

There are no signs of field modification.

Meeting edge protection, gasketing, and edge seals, where required, are present and intact.

Signage on the door does not exceed 5 percent of the door face area, is attached with adhesives only rather than mechanical fasteners, and is not affixed to fire protection glazing.

Clearance under the door does not exceed 3/4 inch, or 3/8 inch where the bottom of the door is more than 38 inches above the finished floor (NFPA 80, 6.3.1.7).

The operational test

The visual checks are only half of it. The door then has to perform: released from the fully open position, the self-closing device has to close the door completely, and the latching hardware has to secure it in the closed position without anyone touching it.

On a pair of doors with a coordinator, the inactive leaf has to close before the active leaf so that the astragal and latching engage in the right sequence.

Auxiliary hardware must not interfere with operation. Hold-open devices that are not part of the listed assembly, including wedges, kick-downs, and hooks, are deficiencies. If a door needs to stay open operationally, the correct answer is a listed hold-open released by the fire alarm system, not a doorstop.

Rolling steel fire doors and the drop test

Rolling and sliding fire doors are inspected and drop tested rather than simply operated. Two drop tests are performed: the first verifies that the door closes fully under its own weight with the bottom bar resting on the sill, and the second verifies that the automatic-closing device was correctly reset after the first.

Average closing speed is expected to be not less than 6 inches per second and not more than 24 inches per second (DASMA Technical Data Sheet 271, restating NFPA 80). Too slow suggests binding or a mechanical problem; too fast is a hazard in an occupied opening.

Before the drop test, the pre-test inspection covers the curtain, slats, endlocks, bottom bar, guides, hood, and the release mechanism. Fusible links and their cables or chains must not be painted or coated with dust and grease, and cables must not be kinked, pinched, or twisted. Deficiencies found in the pre-test inspection should be corrected before the door is dropped, not after.

One practical note: fusible links are not heat tested in the field. They are inspected and replaced on the manufacturer's schedule.

The written record

The requirement that owners most often cannot satisfy is documentation. NFPA 80 requires that a written record of the annual inspection and testing be maintained and made available to the authority having jurisdiction.

A usable record identifies each opening, its location, its label information, the deficiencies found, and the date of inspection. If a deficiency was corrected, the record should show what was done. Building a first-year inventory of openings takes longer than the inspection itself; every year after that is faster because the inventory already exists.

Common questions

Which buildings need annual fire door inspections?

Fire door assemblies are required to be inspected where they are installed as part of a required fire-resistance-rated assembly. In practice that covers most multi-tenant commercial buildings, hotels, schools, healthcare and assisted living, warehouses with rated separations, and any building with rated corridors, stair enclosures, or occupancy separations. Your authority having jurisdiction is the final word on scope.

Can our maintenance staff do it?

The inspection must be performed by individuals with knowledge and understanding of the operating components of the type of door being subject to testing. Many owners prefer a third party because the written record then comes from someone with no stake in the outcome.

What happens if a door fails?

Deficiencies are to be corrected without delay. Most are minor: a missing fastener, a painted label that needs a replacement label from the manufacturer, hardware that needs adjustment, or a hold-open that needs removing. Doors with holes, breaks, or field modifications sometimes require the door or frame to be replaced, because the listing cannot be restored by repair.
